分组 allow_image_generation=false 时，Codex /image 技能通过
namespace 工具声明（type=namespace, name=image_gen）和 Responses
Lite 的 input additional_tools 绕过了现有生图意图检测——现有逻辑
只识别 tools[].type == "image_generation" 这一种形式。
在 gjson 版（openAIJSONToolsContainImageGeneration）和 map 版
（hasOpenAIImageGenerationTool）检测中增加 namespace image_gen
识别；同时扫描 input[].type == "additional_tools" 中嵌套的工具
声明，覆盖 Responses Lite 格式。

refactor(gateway): defer OpenAI request map decoding
...
Keep the Responses hot path on raw request bytes until complex mutation or retry branches need a decoded map, reducing large-body retention under high concurrency.
